چکیده

We note with interest Rolim and colleagues’ cross-sectional serosurvey of residents of Tejuçuoca and Banabuiu in Ceará, Brazil to provide evidence of exposure to Burkholderia pseudomallei , the causative agent of melioidosis. 1 Although there is definitive evidence of culture-confirmed melioidosis in that region, 2 we caution against the use of an unvalidated enzyme-linked immunosorbent assay (ELISA) such as that used by Rolim and others to provide evidence of widespread exposure. Although all serological tests are of relatively limited value for the diagnosis of melioidosis in patients living in areas where melioidosis is endemic (primarily because of seropositivity in the healthy population), the indirect hemagglutination assay (IHA) is widely accepted as the serological test of choice. The diagnostic performance of the IHA has been defined by several studies that used different cutoffs to take account of variable rates of background seropositivity. In the endemic region of Australia, the diagnostic sensitivity of an IHA titer of ≥ 1:40 was 56% at the time of admission (using culture as the gold standard), with evidence of subsequent seroconversion on serial IHA testing in 68% of patients who were initially IHA negative. 3 In northeast Thailand, the diagnostic sensitivity of the much higher IHA titer of ≥ 1:160 was 72% and the specificity 64%. 4 Other assays have been evaluated but have only been found to offer a marginal improvement on the IHA. 5

Use of antigens derived from Burkholderia pseudomallei, B. thailandensis, and B. cepacia in the indirect hemagglutination assay for melioidosis.

The serological diagnosis of melioidosis is carried out using the indirect hemagglutination assay. We looked at the reactivity of sera from culture-proven cases of melioidosis from north Queensland against antigens derived from Burkholderia pseudomallei, B. thailandensis, and B. cepacia. Cross-reactivity between sera from culture-positive cases of melioidosis and B. thailandensis was demonstrated.

Is Melioidosis a One Health-Neglected Disease in Iran?

According to the several scientific resources, Iran is considered to be among the melioidosis-endemic regions of the world; this is in stark contrast to the domestic stance in Iran, where the risk of melioidosis is speculated only as an emerging infectious disease in a non-endemic area. Recently, we have had devastating flash floods in the most provinces of Iran; the changing of soil structure ...

Evaluation of a Latex Agglutination Assay for the Identification of Burkholderia pseudomallei and Burkholderia mallei

Cases of melioidosis and glanders are rare in the United States, but the etiologic agents of each disease (Burkholderia pseudomallei and Burkholderia mallei, respectively) are classified as Tier 1 select agents because of concerns about their potential use as bioterrorism agents. A rapid, highly sensitive, and portable assay for clinical laboratories and field use is required. Our laboratory ha...

Recombinant truncated flagellin of Burkholderia pseudomallei as a molecular probe for diagnosis of melioidosis.

Current serological tests for melioidosis, using impure or uncharacterized cell antigens from Burkholderia pseudomallei, have problems in detection sensitivity and specificity. Therefore, we designed and expressed the recombinant flagellin (truncated at both the N- and C-terminal ends), and used the antigen to develop an indirect enzyme-linked immunosorbent assay (ELISA) to diagnose melioidosis...

Clinical features that affect indirect-hemagglutination-assay responses to Burkholderia pseudomallei.

Melioidosis, a disease endemic to northern Australia and Southeast Asia, is caused by the soil saprophyte Burkholderia pseudomallei. The indirect hemagglutination assay (IHA) is the most frequently used serological test to help confirm exposure to the causative organism. However, despite culture-confirmed disease, patients often have a negative IHA result at presentation and occasionally fail t...
